Pursuant to the provisions of Articles 23 and 24 of the Act of April 15, 2011 on medical activity (Journal of Laws 2011, No. 112, item 654, as amended), the organizational regulations are established as follows:
I. GENERAL PROVISIONS
§ 1. These Regulations define the internal organization of the medical practice of dr. Albert Jeznach § 2. Albert Jeznach runs a medical practice under the name Indywidualna Praktyka Lekarska Albert Jeznach, 05-082 Latchorzew, Gen. Zygmunta Padlewskiego Street, No. 9, hereinafter referred to as “medical practice”.
II. GOALS, TASKS, TYPE AND SCOPE OF PROVIDED SERVICES
§ 3. The purpose of the medical practice is to provide health care services by performing activities aimed at preserving, saving, restoring or improving health, as well as other medical activities resulting from the treatment process or separate regulations governing their performance. § 4. The task of the medical practice is to provide health services in the field of pain management, in particular:
- health assessment and prevention,
- diagnosis and treatment of diseases,
- conducting consultations and resulting diagnostic tests,
- issuing medical opinions and certificates. § 5. Provision of services is carried out in the form of consultations using electronic data transmission channels.
III. ORGANIZATION OF THE PROCESS OF PROVIDING SERVICES
§ 6.
(1) The activities of the medical practice shall be managed and directed by the doctor who established it, i.e. Dr. Albert Jeznach. (2) The doctor referred to above shall be responsible for the proper functioning of the practice in accordance with applicable regulations, and shall represent the practice externally. § 7. (3) Provision of health services to patients within the scope of medical practice shall be carried out after prior registration of the patient for a certain day and time from the scope of routine working hours of Doctor Albert Jeznach. 4. the days and hours of routine work of the medical practice are made available through the websitej www.jeznachmd.com 5. patients are registered and admitted on a first-come, first-served basis. 6. Patients can be registered for an appointment by: (a) through the online registration system available through the websites:

- https://placowki.mp.pl/unit/index.html?id=841789 (b) direct registration by a practicing physician. 7 The telephone and on-line registration system (eMPendium calendar) is run through an external company - Medycyna Praktyczna Spółka z Ograniczoną Odpowiedzialnością Sp. K. with its registered office in Kraków (30-510) ul. Rejtana 2, entered in the Register of Entrepreneurs of the National Court Register kept by the District Court for Kraków Śródmieście in Kraków, XI Economic Department of the National Court Register, under the number KRS 0000470072, NIP 6790044689, REGON 350023040, phone: +48122934000, fax: +48122934010, email: sekretariat@mp.pl. with which a separate agreement for the provision of these services has been concluded.
- eMPednium calendar service regulations are available to the patient on the Practical Medicine website.
- cancellation of an appointment can be made by telephone at the telephone numbers specified in §7 pt. 3a or through the online registration system. During the online registration, the patient will receive by e-mail instructions to cancel the appointment electronically. (10) Cancellation of an appointment should be reported as soon as possible, but no later than 24 hours before the appointment. 11 The medical practice has the ability to issue a ZUS-ZLA. The legitimacy of issuing a ZUS-ZLA is always based on the course of the diagnostic and therapeutic process and is at the discretion of the doctor in charge of the medical practice.
IV. MEDICAL DOCUMENTATION
§8. a. In the medical practice, medical records are kept only in electronic form b. The program eMPendium EDM - Electronic Medical Record provided by Medycyna Praktyczna Spółka z Ograniczoną Odpowiedzialnością Sp. K. with its registered office in Kraków (30-510) 2 Rejtana Street is used to keep medical records. §9.
(1) Medical records shall be made available: (a) in the form of an electronic document in pdf format sent to the e-mail address provided by the patient during registration (2) On the basis of the Act on Patient’s Rights and Patient’s Ombudsman of November 6, 2008. (Journal of Laws of 2009, No. 52, item 417, as amended), it is established that there is no fee for providing medical records in the form of an electronic document.
V. SECURITY AND PROCESSING OF PERSONAL DATA
§ 10.
(1) The medical practice shall have a Security Policy for the Protection and Processing of Personal Data in accordance with current legislation, including the “General Regulation on the Protection of Personal Data (Regulation of the European Parliament and the Council of the EU) No. 2016/679 of April 27, 2016.” 2. documents related to the Security Policy for Protection and Processing of Personal Data are available on the medical practice’s website www.jeznachmd.com
VI. FEES FOR PROVIDING SERVICES
§ 11.
(1) Health services provided by: medical practice shall be provided against payment. (2) There shall be no provision of services financed by public funds under a contract with the National Health Fund. (3) The fee for providing health services (medical consultation) during the hours of routine work of a medical practice is: first visit - 300 zlotys (three hundred zlotys) lasting a maximum of 45 minutes, subsequent visit - 200 zlotys (two hundred zlotys), lasting a maximum of 30 minutes, issuing a prescription for permanent drugs - 80 zlotys (eighty zlotys), home visit - from 500 zlotys depending on the location (from 500 zlotys) (4) The fee for the provision of health services (medical consultation) outside the routine working hours of the medical practice (so-called urgent visit) is determined individually before the start of the visit. (5) In special cases, the fee for providing health services within the routine working hours of the medical practice may be determined individually, in which case the fee will not be more than 80 PLN (eighty zlotys). (6) The fee for medical consultation does not include the possible costs of additional tests (laboratory, imaging, consultation of other specialists). (7) Payment for health services in medical practice can be made before or after the visit. 8. The following forms of payment are available at the medical practice:
- cash
- bank transfer or BLIK
VII. FINAL PROVISIONS
§ 12.
(1) A patient who has reservations about the quality of services provided or the timing of their performance may file a complaint. Complaints are accepted only in writing within a maximum period of 7 days from the date of service provision. The complaint will be considered within 30 working days from the date of its receipt. (2) In matters not regulated by these Organizational Regulations, the provisions of:
- the Act on Medical Activity of April 15, 2011 (Journal of Laws of 2011, No. 112, item 654 as amended),
- the Act on Patient Rights and Patient Ombudsman of November 6, 2008. (Journal of Laws of 2009, no. 52, item 417 as amended).
- General Regulation on Protection of Personal Data (Regulation of the European Parliament and the Council of the EU) No. 2016/679 of April 27, 2016.
(3) The Regulations shall come into force as of 2019.02.07, updated 2023.12.03
